During your first appointment, PACIFIC PHYSIO will spend time getting to know you- including more about your condition, circumstances, and what you'd like to achieve.
Next, we'll assess your physical function-looking at things like your walking, transfers, balance, and coordination. We'll make sure you always feel safe and comfortable while doing these.
Then, we'll talk to you about our assessment findings. We'll discuss what your goals are and what you would like PACIFIC PHYSIO would like to help you achieve. From there, we will together set a plan of action and how we can achieve this together. PACIFIC PHYSIO will always consider your preferences and remain honest and realistic about time frames and expectations.
During this first appointment, we will also consider you, as a person, outside of the "physiotherapy box". If we feel like you could benefit from input from other clinical experts, we'll discuss this with you and can help you organise referrals to trusted professionals. This may include community nurses, Occupational Therapists, Dieticians, Speech Language Pathologists, etc.
For your follow-up PACIFIC PHYSIO appointments, we'll work alongside you to target your therapy goals. This will look different for every person but may include things like balance training, specific stretch or strength exercises, improvement or maintanance for your function (walking, transfers, etc), cardiovascular program, or carer training.
PACIFIC PHYSIO will constantly monitor your progress and adjust your program as necessary, always considering your needs and preferences.
Family members or carers are more than welcome to participate in your assessment and therapy.
Good communication is vital amongst your support team- so with your permission, we'll ensure we stay in contact with those involved in your care, such as your GP, Neurologist, Support Team, and other Allied Health Professionals.
